For membership in Phi Theta Kappa, you
must have:
-
a cumulative GPA of at least 4.50, and
-
at least 12 hours of completed college level
course work.
If you meet these requirements, please submit
this completed form along with your one-time membership fee ($65, cash
or check payable to DACC) to

freedom, and privileges of my country (International students attending
college on a United States visa and who possess full right of citizenship
of their country are eligible for membership).

I understand that there is a required cumulative
GPA (4.50 for admission, 4.25 to maintain) for membership in Phi Theta
Kappa and agree to notify my chapter advisor (Lori Garrett) immediately
if at any time I fail to maintain my chapter's maintenance GPA.

I solemnly promise to uphold the standards
of Phi Theta Kappa, and to make this object and aim foremost in my mind,
and I do solemnly pledge allegiance to my fellow members and promise to
aid them in all worthy endeavors.
