Stafford Loan (FFEL)
Request Form
Please read the
following
information and answer the questions below. Once completed submit
the form
to the DACC Financial Aid Office (FAO), in person, by mail or
fax.

Once
submitted you
will be notified of
whether or not you are eligible to be certified for a Stafford Loan
this Award Year. If
eligible you will receive
a letter with further instructions on how to apply and complete the
loan process. There will be several lender's Web sites listed on
this letter you may use one of them if you like. You
may use any lender who participates in the FFEL Program.
If you do not see your lender listed you should check with your
lender to see if they have
an online application option.
Note: 34
CFR Section 668.202(e)(1)
of the Federal Family Education Loan (FFEL) Program provides that a school
may refuse to certify a FFEL Stafford or PLUS loan application or
may
reduce the borrower's determination of need for the loan if the
reason
for that action is documented and provided to the student in writing
provided
the determination is made on a case-by-case basis. If
you are not eligible for a Stafford loan this Award Year you will be
sent a notice indicating the reason.
